Tiffany & Co. 9000 Theme Request #2

I have a Blackberry Bold 9000 OS 4.6 and I would like another Tiffany & Co. theme to go with my new cover that i got for my phone.

I would like it in ZEN format with the ORIGINAL icons. I would like SMS and MMS together please (not separate icons) and 6 ICONS on the ZEN screen.

I would like this picture for the ZEN ICON SCREEN and LOCKSCREEN:

(Please no other colors around the picture, just the plain white as in the picture)

I would like this picture for the INCOMING CALL SCREEN:
[Only registered and activated users can see links]
with the number calling in BLACK font.

I would like the MAINSCREEN to be BLACK with WHITE FONT.
